Important Notice from the Financial Aid Office
NEW IDENTIFICATION REQUIREMENTS FOR OSAP & OUT OF PROVINCE LOANS – EFFECTIVE MAY 1, 2008
Due to revised government regulations, effective May 1, 2008 we can no longer accept Sheridan student ID cards or LCBO Age of Majority Cards (BYID) as an acceptable form of photo identification in order to obtain or negotiate loan/grant certificates.
Examples of acceptable photo ID include:

A valid driver's license;

A Canadian passport
-
A provincial health card with a photo; or
-
A permanent residency or citizenship card
AND
OR
- Official Government of Canada document
(eg. Canada Revenue Agency Notice of Assessment, Canada Pension Plan Statement of Contributions, confirmation for Social Insurance Number from Service Canada)
Note: T4's, T4A's, T5, Records of Employment are NOT acceptable
